Referrals are accepted for any patient with life limiting illness who meets the hospice's specialist palliative care criteria.

Our services may be appropriate at any stage of an individual’s illness. We are happy to discuss potential referrals directly over the telephone with the referrer as necessary.

Written medical confirmation may be requested and a member of the hospice team will visit the patient to make an initial assessment and offer appropriate services.

The team can usually be telephoned between 9am and 10.30am and 4pm to 5pm. Secretarial staff will take a message outside these hours. Please see below for contact details.
