Referrals are accepted for any patients with life limiting illness. The service’s principal focus is providing nursing care at the “End of Life”, for example when the Liverpool Care Pathway is in place.

This is especially pertinent when the patient has expressed a preference to be at home for their last days to weeks, and their carer is supportive of this choice.

Additionally, the service is able to provide “respite” visits of up to 5 hours per day, or at night, to give carers a much needed break from their caring role. Respite is offered for an agreed period of weeks and reviewed regularly with the patient and their family.

Written medical confirmation may be requested and a member of the hospice team will visit the patient to make an initial assessment and offer appropriate services.
