We believe that your care should be as individual as you are, that’s why we provide support 24 hours a day, 7 days a week —whether it’s over the phone or visiting you at home. Our community services offer greater choice and flexibility, so no matter when, or where, we are here for you.

Hospice at Home

Our Hospice at Home services aim to enable patients to be cared for, and to die, at home if that is their preference.

We provide a range of practical care and support. This could involve helping those caring for you to have an undisturbed night’s sleep, providing personal care, giving emotional support to you, and your loved ones, or providing much needed respite. We provide the highest standard of care to enhance your quality of life, whilst supporting your carers and loved ones.

Specialist palliative care

Our community team provide support to those who have received a diagnosis of advanced life limiting illness.

We support with the facilitation of end of life care at home. This includes services such as; advanced care planning – with a review of symptoms and the development of management plans, emotional support to you, and your loved ones, as well as signposting and engagement with external providers including GPs, District Nurses, and hospital palliative care teams.

Respite services

Our respite services provide support for carers who are looking after loved ones in the final stages of their life.

The service consists of visits to enable carers – defined as anyone who looks after a family member, partner or friend –  to rest and take a much-needed break.

We also provide carers with the resources, knowledge, skills and support they need to carry out their caring roles.

Symptom control

Our aim is to help you to live your life as you want to live it. We will work with you and those treating you to get the best support, treatments or medicines so that you can.

Promoting independence

It’s important for patients to retain a sense of control so our therapies team will work with you to find out what matters most and help you achieve it.

Emotional wellbeing

We understand that your emotional wellbeing – and that of those closest to you – is just as important as your physical wellbeing, and our care plans reflect this.

Welfare support

The financial burden caused by a terminal illness can be worrying too, so we can signpost to services that offer the appropriate advice and guidance.
